プログラム参加同意書

私は、本プログラムに参加するにあたり、自己あるいは他者の行為・不行為・不注意により損失・損害・障害に至る危険がある事を認識しインストラクターの判断及び指示を厳守します／In participating in this program, I recognize that there is a risk of loss, damage, or injury due to my own or others' actions, inactions, or carelessness, and I will strictly follow the instructor's judgment and instructions.

私は、プログラム中に事故・怪我・発病が万一発生した場合、応急処置などについて、その一切を主催者に委ねます。 なお、その際発生した身体及び精神的な傷害ならびに医療費・その他の費用について、主催者・インストラクター・施設側に対して責任を追求したり損害賠償名などの請求を致しません／In the unlikely event that an accident, injury, or illness should occur during the program, I will entrust all of the first-aid measures to the organizer. In addition, regarding physical and mental injuries, medical expenses, and other expenses that occurred at that time, we will not pursue responsibility or claim damages from the organizer, instructor, or facility side.

私は、プログラム中他人に怪我を負わせた場合、その責任は参加者自身にあり、主催者・施設側いずれに対しても怪我等に関する費用や損害賠償等の請求を致しません／If I injure someone else during the program, the responsibility lies with the participant, and I will not claim expenses or compensation for damages from either the organizer or the facility.

私は、プログラム中自らの故意・過失によって器物等を破損・損傷等をした場合、主催者に対しその損害金額の請求に従い賠償するものとします／If I damage or damage the equipment during the program due to my intention or negligence, I will compensate the organizer for the amount of damages.

私は、クラブ/ロッカー内での盗難や紛失については、自責・他責に関わらず主催者・施設側に対して一切責任を問いません／I will not hold the organizer/facility responsible for any theft or loss in the club/locker, regardless of whether it is my responsibility or someone else's fault.

私の現在の健康状態は、プログラムの参加に関して問題ありません／My current health condition is in order to participate in the program

私はSURFCITYのプログラム参加にあたり、上記の事についてすべて同意したうえで体験の申し込みをします／Upon participating in the SURFCITY program, I agree to all of the above before applying for the experience.
